As required by Federal and State law, First Choice Medical Transport must protect and secure all the health and identifiable information we have regarding our patients. First Choice is not permitted to use or disclose any of our patient’s information unless it is allowed by the patient or the POA. We are required to inform you of your rights and our legal duties with respect to your health information with this statement.
EFFECTIVE DATE: JANUARY 1, 2015
As allowed by law, First Choice Medical Transport will use and disclose patient health information for treatment, payment or health care operations.
Treatment: First Choice may use or disclose patient health information to hospital and nursing home staff, or other health care providers who contribute to patient care.
Payment: First Choice May use or disclose patient health information to create a bill, or to obtain medical necessity and pre authorization for the services provided by us. We may not use or disclose this information to the patient’s individual health plan or health care provider, including Medicare and Medicaid, without consent.
Health Care Operations: First Choice may use the patient’s health information for conducting quality assessment and improvements to our business/operations.
